  1. Financial assistance is sought in good faith.
  1. Any application made is made in the knowledge that all accounts, receipts or documents must be supplied to The Code 9 Foundation committee for verification and recording.
  1. Applications must be made in advance of the service sought and not retrospectively.
  1. When the service requested has been agreed to the applicant(s), where applicable, must be present at the location the service is required. This ensures support for the affected person as well as the service provider.
  1. Issues/problems/dissatisfaction with the service provider are to be reported to the providers or providers governing body.
  1. The Code 9 Foundation accepts no liability for breakage or damage to the applicant(s) personal or private belonging(s) as a result of services provided by the third party.

  2. Any decision for approval or denial is made by three committee members and all decisions are absolute and final.
